I need to have the usb charger connection replaced, where can I have that done?

My phone(Nokia 1020) to won't charge because the cord keeps coming out of of the connection...it won't stay in the right spot for it to charge. I guess it's worn out. How can I have it replaced? (And get the battery replaced too?) Any help is appreciated
